I didn't proof read a piece of direct mail properly when I worked for WH Smith.
Only when about 2 million postcards had been printed did I realise (thankfully before they were sent). They all had to be pulped & reprinted at a cost of about £300,000.

I told my boss, offered to resign. Thankfully she didn't take me up on my offer.
