While I'm complaining...There used to be a thing called the National Patient Safety Agency, and they had some people who knew what they were talking about, and they collected incident data from NHS Trusts in a way that was straight forward and not at all bothersome once you'd got it set up.
That was binned and replaced with 'learning from patient safety events' run by NHS England and it just doesn't work. It's more difficult to manage, it makes incident reporting more difficult for people reporting incidents, the data coming back out is shite and lots of it just doesn't work. It's incredibly annoying to have had something that worked well replaced with something shonky.
