Is it done by hand, or do you have some clever software to help with the movement?
I only ask as it reminds me of a Budweiser promotional thing where you could animate a photo and make it look like it was talking/blinking.

It's just done with the PS liquify filter..!!
set on rigid...brush the size of the monkeys head.
smaller brush (nose and mouth) at the top of the upward head movement to try and get some perspective.
Some bits of extra jiggerypokery on the eyes and mouth too.
