'Madam'
Hate it. Hate it with a passion. Since my teens I have been subjected to this, and most recently by a security guy in the student union. Oh how I long to be called 'miss'.

But "miss" is sexist.
Apart from the fact that the male equivalent is "Mr." you also have to take into account that some women find "Miss" to be unnaceptable and demand "Ms".
Heck, don't even get me started on man/boy woman/girl/lady and the other bullshit that makes talking to a woman fraught with problems.
