I can count on one hand the number of times it has happened to me.
Fortunately, I am not inbred, and therefore have the normal number of fingers on my hands.

It happens
In my (not extensive but reasonable) experience, these things happen, depends on the girl from what I can tell.

Worst time ever was when one girl said sorry every time it happened. Puts you off when it's each and every stroke I can tell you.

Found out that she'd also been with one of my mates and the exact same thing happened. :/
