Done a Windows install on a laptop, but not plugged the charger inso just as it was about to finish, the battery ran out. Start again.

Similarlyone of our techies plugged a new server into the mains without realising it was set to USA 110V, putting 240V into it resulted in a spectacular bang and bits part-embedded into the server room wall.
