I fell off a bike when I was 12 and did myself some serious damage
Split knee (4 stitches), broken teeth, hands cut to ribbons and my forehead still bears the evidence in the form of a bump on the bone.

I broke my nose when I was four.
I accidentally whacked it into a metal bar. Blood went everywhere.

I've been lucky in that I've never had anything major happen
Broken foot, broken funnybone, broken leg (18 months old), cuts and scars

A friend of mine still has a scar from when she was about 5, where she ran past a radiator, stumbled, and went cheek first into it.
