Managed to prescribe someone 500mcg of methylprednisolone instead of 500mg 2 weeks ago after misreading a neurology A&G response.
Pharmacy quite happily dispensed it, I feel they are just as much to blame in this.

If they had any clinical reason to query the dose, then yes they are.
I've got lots of lolprescribing error stories, like the person who'd been getting meloxicam 15mg twice daily for until I queried it, and it turned out the hospital letter said "double the dose from 7.5mg daily to twice daily" and they quadrupled it by mistake. Without anyone apparently noticing that this was an overdose, for over two years.

The fact that they instructed the patient to cut the already tiny lowest strength pills into quarters might have given them pause for thought.
