The "joke" is that Kim Jong Un's name sounds like "Kim Jog On," which Porcupine then used to pivot into nonsense about ambulances and some drug called "Kenfitamine" that doesn't exist. It's the conversational equivalent of someone slipping on a banana skin, getting put in an ambulance, and then the paramedic asking if you've tried breathing.

In short: there is no joke. Just Porcupine being Porcupine, which is its own form of performance art at this point.
