in pubs etc . . . .
me 'pint of Coke please'
Them 'We only have Pepsi, is that ok?'

has anyone ever in the history of anything ever said 'oh no. It's Coke or nothing'

I know they probably have to say it on pain of being stabbed, but really . . . . .

it is important the other way around though
if you don't say "a pint of cola please" (about a quid, straight out of the soda tap) but instead say "a pint of coke" (about three quid, made up of two of those tiny bottles, not quite a full pint) you're stuffed

Yes, me.
All the time.

If they only have Pepsi, I will refuse and order something else instead.

Coke and Pepsi taste very different. It's perfectly possible to like one, but not the other. I'm sure there are many other people who prefer Pepsi...

No it has to be "coke"
pepsi is simply not as good at removing rust from metal

Not had anyone refuse it
However I knew a chap that regularly came into the bar I worked in to order drinks for his boss (landlord) and all their cronies. He had to ask me everytime if we had pepsi for the boss (we never did) as that is what he drinked.

He accepted coke every single time.

My friend sells coke to his 'clients'.
I don't think they'll take Pepsi.

I worked at McDonalds in the early 90s.
Before they sold Coke, so..
"Big Mac and a Coke please"
"That's McDonald's cola, is that ok?"

It's a legal issue
If somebody asks for Coke, the bar staff / waiter HAS to say that it's not 'Coca-Cola' (the association is there).

The same if they order Pepsi (they have to say that they only have Coke.

And yes, I've had a bloke refuse Coca-Cola on the grounds that it's manky.

it tastes far sweeter than coke, i really don't like it.

^this, very much
If a pub offers me Pepsi, I'll refuse it and have water instead.

For me, it really is (diet) Coke or nothing.
