Always WACOM..
A4 in size the A5's are too small.. have a look on ebay you can pic up some older USB WACOMs for good money.
Any of the intuos ranges are good, the newer the better it terms of accuracy and pressure levels. Also gettiing spares for older ones (nibs) can be a bit of a pain.
If money is not an issue WACOM graphire.
EDIT: Ignore the Graphire comment intuos 3 is what you want A4 new you'll be paying about £350 squids...
Although from experience I have discovered a serious flaw with the Intuos 3 it is NOT coffee proof, spill something on it and you may well break it, the Intuos 1 and 2 are pretty much water proof, a design tool that is not Coffee proof, thats crazy

I find the larger size one enable
you to make more acurate shapes, especially curves as you can use traditional sketching techniques to get the curves, which I why I got an A3, but then I am a twunt.

I can certainly see advantages to the larger ones, i suppose it's
limited by budget, but also the general size the images you're used to drawing (on paper).
I just happened to obtain an Intuos3 A5 for free and I'm used to drawing small pics on A4 paper...suits me nicely
