although... I hate to say it, but if the NHS implement such a thing, it wouldn't guarantee death. I've seen people shot in the head but still survive. (a great epidsode of Cops (or something) showed them answering a call where someone had shot himself in the head as a suicide attempt, but only managed to blow his cheek and ear off.)

Might I suggest instead shooting, or hammering a wide stake where the back of the skull meets the neck, in what would be a fairly decent attempt at severing the brainstem. In the majority of cases, the person would die quite quickly, reducing the possibility that errors in diagnosis will occur.
